Some people say that a picture is worth a thousand words. The field of management often uses a "rich picture" systems methodology, "an innovative tool that encapsulates knowledge relevant to strategic reform". It is often described in the management literature as a "soft systems methodology" for linking hard and soft facts in a cartoon-like representation to illustrate a complex problem simply and clearly. The following research is presented using the rich picture methodology to capture the current long-term care and long-term services and supports (LTSS) crisis. The picture and narrative rely on expert research from the past and present, as well as on one-on-one open-ended interviews with key stakeholders in the fields of disability, long-term care, and health care. The setting for the rich picture is the ocean, with the current LTSS ship heading toward an iceberg that represents the barriers and challenges to systems reform. The "cast" for this rich picture will provide the substantive descriptions and body of research and analysis about the barriers and challenges of navigating through the current system of LTSS. The presentation of the research is purposeful, so that the reader and the researcher can begin the voyage together with a snapshot of the problem. The purpose of this research is to produce new knowledge and understanding of current experience with and future need for affordable LTSS for people with disabilities. This research on the State of LTSS Financing and Systems Reform is the first part in a five-part series that will tell the story of the current LTSS system to set the stage for the exploration of future market demands and current gaps in supply; to explore promising state practices and challenges; and to picture what the 21st century''s comprehensive, consumer-responsive system might look like and make policy recommendations. The research is based on five assumptions. First, people with disabilities, whether young or old, desire and deserve choices when seeking assistance with daily living that maintains their self-determination and maximum dignity and independence. Second, the current financing mechanisms (public and private) will become unsustainable in the near future without significant reform. The system must be affordable to all Americans regardless of income levels and must consider opportunities to leverage public and private support in new ways without impoverishing beneficiaries. Third, there is an opportunity with the changing demographic picture of the United States to explore the possibilities of a universal approach to the design and financing of services and supports that is responsive to individuals under the age of 65, as well as seniors with disabilities, without sacrificing individual choice and flexibility. Fourth, formal and informal caregiving must be sustained, examining family needs and workforce recruitment and retention challenges. Fifth, the approach to quality must examine consumer direction and control of resources in addition to traditional external quality assurance mechanisms.

Problem-solving is an important part of our lives. Dialectical Networking Thinking (DNT) is a soft-systems methodology aimed at complex problem-solving. The modern (dynamic) business is a demanding and complex process. It requires permanent learning, innovating, and adapting to the requirements of demanding customers. The latter must be able to face their challenges and change themselves accordingly, first of all by innovating their own values and knowledge, especially concerning holistic complex problem-solving in business. DNT enables us to do this. A key issue namely keeps surfacing: what is the correct way to tackle mastering and solving complex problems, in order for objectives and solutions to be high quality and sustained. In daily practices, unfortunately, one tends to underestimate the complexity of problems one faces and tries to solve them (wrongly and irresponsibly) one-sidedly. This approach causes new problems to emerge, perhaps even more intricate ones. This problem can be avoided by using DNT, following the steps briefed above. DNT can enjoy evolution to help solve complex problems, if all preconditions are met for its application. They include, along with knowledge and motivated teams, an innovation-seeking organization and its prevailing culture. In the best-case scenario, every OBS member would accept DNT because such a capacity provides the upper hand in modern competition. On the other hand, limitation to single disciplines, lack of co-operation on an interdisciplinary basis and in teamwork will unavoidably fail in the modern innovative economy.

Over the past two decades, it has been recognized that advanced image processing techniques provide valuable information to physicians for the diagnosis, image guided therapy and surgery, and monitoring of human diseases. This book introduces novel and sophisticated mathematical problems which encourage the development of advanced optimisation and computing methods, especially convex optimisation. The authors go on to study Steffensen-King-type methods of convergence to approximate a locally unique solution of a nonlinear equation and also in problems of convex optimisation. Real-world applications are also provided. The following study is focused on the design and testing of a Matlab code of the Frank-Wolfe algorithm. The Nesterov step is proposed in order to accelerate the algorithm, and the results of some numerical experiments of constraint optimization are also provided. Lagrangian methods for numerical solutions to constrained convex programs are also explored. For enhanced algorithms, the traditional Lagrange multiplier update is modified to take a soft reflection across the zero boundary. This, coupled with a modified drift expression, is shown to yield improved performance. Next, Newtons mesh independence principle was used to solve a certain class of optimal design problems from earlier studies. Motivated by optimization considerations, the authors show that under the same computational cost, a finer mesh independence principle can be given than before. This compilation closes with a presentation on a local convergence analysis for eighthorder variants of HansenPatricks family for approximating a locally unique solution of a nonlinear equation. The radius of convergence and computable error bounds on the distances involved are also provided.

The cirrhotic liver is significantly more vulnerable to injury. This vulnerability is particularly evident after the advent of portal hypertension. In the normal liver, up to 75 percent can be resected without decompensation. By contrast, in the cirrhotic liver with portal hypertension, liver decompensation can occur even after less than 50 percent of the liver is resected. This book begins with an attempt to provide insights into this unanswered puzzle. The normal liver is soft and elastic. By contrast, the cirrhotic liver is inelastic and rigid. Flow in rigid tubes is governed by the Hagen-Poiseuille law. The law dictates that pressure gradient increases exponentially by the fourth power of the radius of the rigid sinusoid. This makes the cirrhotic liver virtually incapable of accommodating any increase in flow. The increase in pressure gradient is equivalent to the fourth power of the vessel radius. The first chapter explores the major new insight that decompensation in the cirrhotic liver is related to the loss of elasticity and the development of vessel wall rigidity, which thereafter subjects flow in the hepatic sinusoids to the Hagen-Poiseuille law. The rest of the book addresses portal hypertension in its clinical context. It invites the reader to survey multiple avenues associated with portal hypertension, including idiopathic non-cirrhotic portal hypertension, INCPH and non-alcoholic fatty liver disease, NAFLD. It also includes chapters on relationships between portal hypertension, the gastrointestinal system and the renal system. In general, the book provides several new insights into portal hypertension.

The main focus of the research described in the opening chapter of Salt Marshes: Formation, Ecological Functions and Threats focuses on the study of the plant heritage of the Fuerteventura salt marshes and their surroundings, together with the analysis of their ecological value, threats to the marshes, and of use and recent management practices. The methodology used predominantly consists of field work, photo-interpretation4, and implementation of a GIS from the information obtained and the database generated. The Fuerteventura salt marshes are of great importance because of their vegetal wealth and role as a refuge for fauna, especially birds and invertebrates. The subtropical saltmarshes of Brazil are located in the South of Brazil, between the north-central coast of São Paulo state and the central coast of Santa Catarina state, and have been studied over the course of many years by the researchers M.R. Bornschein and B.L. Reinert, resulting in their recognition as a new ecosystem. Despite several ornithological researches in these marshes, little is known about detailed aspects of their ecology and other groups of animals. Thus, the authors provide a general description of this recently recognized ecosystem and the ecological influences over the species that live in these marshes, resulting in the fragile ecological balance point of Borschein-Reinert, and propose a mathematical index to this balance. Salt marshes develop in estuaries where there is reduced wave action, which allows for a source of sediment and suitable conditions for marsh plants to grow. The vegetation generally provides a structural habitat on featureless soft-sediment bottoms, and so it is utilized by a wide and diverse range of fish and invertebrates as their physical home, food supply and shelter from predators. The authors discuss the way in which species such as insects and crabs demonstrate variable levels of adaptation to life in this intertidal habitat, as well as how species composition and abundance of benthic invertebrates are strongly influenced by changes in the vegetation.

Cannabis sativa is the scientific name for the widely used drug commonly known as marijuana. Marijuana is the most commonly used illegal drug in the United States as well as in many other countries. Marijuana contains tetrahydrocannabinol (THC), which is a mind altering substance that is generally smoked, but also can be taken orally or as oil on the skin in order to receive a "high." New laws regarding the marijuana''s legalisation have led to more questions and increased interest in the effect marijuana has on its users. Chapter One of this book discusses differences across motivation variables and marijuana use. Chapter Two compares the medical necessity of marijuana to the political agenda. Chapter Three examines the evidence for and against the medical use of marijuana, and the effects and implications of its recent legalisation in several states especially with regard to its use by college-age young adults. Chapter Four examines the issues surrounding legalisation and teen use and the policy steps that might be taken to effectively communicate the risks to adolescents amidst an ever more socially acceptable climate. Chapter Five discusses the recent rise of legally sold marijuana, the risks of use, arguments in support of mandating warnings on packages of legally sold marijuana, the likely benefits and costs of mandating warnings, ways of evaluating proposed and actual warnings, and specific recommended warnings. Chapter Six exhibit how and the extent to which federal government marijuana regulation and policy contributes to disparities in arrest, in particular for African Americans of lower socioeconomic standing. This book ends with a commentary on aims to answer the question of whether marijuana and video games can engender "real" addictions in a way that bridges the gap between scientific and colloquial understandings of problem use.
