Asthma - William Berger - Bog - Oxford University Press Inc - Plusbog.dk

Asthma - William Berger - Bog - Oxford University Press Inc - Plusbog.dk

Asthma is a common chronic inflammatory condition affecting the airways and displays a varied phenotypic picture. It is becoming increasingly recognized by healthcare workers and epidemiological studies suggest that along with other atopic diseases, its prevalence is rising. The precise etiology of asthma remains uncertain, but genetic and environmental factors such as viruses, country of origin, allergen exposure, early use of antibiotics, and numbers of siblings have all been implicated in its inception and development. Pathologically it is characterized by inflammation, physiologically by airway hyper-responsiveness (or hyper-reactivity) resulting in reversible airflow obstruction, and clinically by wheeze, chest tightness, breathlessness and cough. It can present in early childhood as well as adulthood, and varies markedly in severity, clinical course, subsequent disability and response to treatment. Exacerbations and symptoms of asthma are the final manifestation of a complex interplay between an array of inflammatory cells and mediators, which cause airway smooth muscle to intermittently relax and contract. Despite greater knowledge surrounding the immunopathological origins of asthma and considerable advances in its management, it remains one of the most important chronic diseases in young adults and poses a significant degree of morbidity throughout all age groups. A minority of patients experience difficulty in controlling asthma and pose significant therapeutic difficulties in specialist clinics. Exacerbations of asthma contribute to significant costs for healthcare systems and are implicated in adversely affecting the quality of life of individuals and their families. Moreover, although asthma deaths have decreased over the past few decades, an appreciable number of deaths still occur each year. Regular anti-inflammatory therapy with inhaled corticosteroids is required in all but the mildest of disease and attenuates underlying airway inflammation and hyper-responsiveness, while bronchodilators are designed to relax airway smooth muscle and prevent bronchoconstriction on exposure to bronchoconstrictor stimuli. Other forms of treatment are required in individuals with persistent symptoms and exacerbations. In recent years several potentially exciting treatments have emerged and are in varying degrees of development. Part of the new Oxford American Respiratory Library, this concise, portable guide provides an essential reference on current, evidence-based medical approaches to diagnosing and managing asthma. This practical volume features chapters on the pathophysiology of the disease with information on the common symptoms and potential triggers. In addition, the text includes discussions of new and emerging pharmacotherapies and complementary treatment therapies, with guidelines for symptom prevention.

Why Study Religion? - Richard B. (laura Spelman Rockefeller Professor Of Religion Miller - Bog - Oxford University Press Inc - Plusbog.dk

Why Study Religion? - Richard B. (laura Spelman Rockefeller Professor Of Religion Miller - Bog - Oxford University Press Inc - Plusbog.dk

Can the study of religion be justified? Scholarship in religion, especially work in "theory and method," is preoccupied with matters of research procedure and thus inarticulate about the goals that motivate scholarship in the field. For that reason, the field suffers from a crisis of rationale. Richard B. Miller identifies six prevailing methodologies in the field, and then offers an alternative framework for thinking about the purposes of the discipline. Shadowing these various methodologies, he notes, is a Weberian scientific ideal for studying religion, one that aspires to value-neutrality. This ideal fortifies a "regime of truth" that undercuts efforts to think normatively and teleologically about the field''s purpose and value. Miller''s alternative framework, Critical Humanism, theorizes about the ends rather than the means of humanistic scholarship. Why Study Religion? offers an account of humanistic inquiry that is held together by four values: Post-critical Reasoning, Social Criticism, Cross-cultural Fluency, and Environmental Responsibility. Ordered to such purposes, Miller argues, scholars of religion can relax their commitment to matters of methodological procedure and advocate for the value of studying religion. The future of religious studies will depend on how well it can articulate its goals as a basis for motivating scholarship in the field.

Changing Worlds - David W.p. Elliott - Bog - Oxford University Press Inc - Plusbog.dk

Changing Worlds - David W.p. Elliott - Bog - Oxford University Press Inc - Plusbog.dk

For the most of the twentieth century, the country of Vietnam has served as a symbol of the bipolar system of rival ideological blocs that characterized the Cold War. As the conflict over communism waned in the 1980s, Vietnam faced the tough task of remaking itself as nation in the eyes of its people and of the world. In Changing Worlds, David W.P. Elliot, a participant in the Aspen Institute''s U.S.-Vietnam Dialogue who has spent the past forty years working closely with the people and government of Vietnam, chronicles the evolution of the Vietnamese state as we know it today. With the collapse of communist regimes in Europe, Vietnam witnessed the dissolution of the cornerstone of its policies toward the outside world. Fearing that a full commitment to deep integration in a globalizing world would lead to the collapse of their own current political system, the Vietnamese political elite made slow, cautious steps to involvement with the larger international community. By the year 2000, however, Vietnam had "taken the plunge" and opted for greater participation in the global economic system, leading to its membership in the World Trade Organization in 2006.Elliott illustrates that the politicians who took a limited approach to international involvement ultimately had condemned Vietnam to a permanent state of underdevelopment. It is only at the turn of the 21st century when the Vietnamese state began to relax its policies toward the international community that the nation began to experience a period of revitalization. Remarkably, these changes have happened without Vietnam losing its unique political identity as many had expected. It remains an authoritarian state, but offers far more breathing space to its citizens than in pre-reform era. Far from leading the nation to be absorbed into a Western-inspired development model, globalization has led to a complex domestic diversification and localization that has reinforced Vietnam''s distinctive identity rather than obliterating it. The culmination of decades of research and cultural exchange, Changing Worlds documents the unique story of the birth of a nation amidst the challenges of the post-Cold War era.
