Three points must strike anyone who has embarked on a study of dementia over a period of time. Firstly, that our conception of the syndrome is in a state of flux. Gone, for instance, in the past decade or two, is the requirement of a chronic, progressive, irreversible disorder for the diag­ nosis. I remember the surgeon who, when I was a student, returned a referral saying he would operate on the man when his dementia got better. Feeling superior, and encouraged by the consultant psychiatrist, we students laughed a good deal at this. Before we finished clerking on that Unit a visiting Professor of Psychiatry had demonstrated the reversibility of the symptoms of dementia in a patient with a rare metabolic disorder. Perhaps ignorance is sometimes an advance on received wisdom. The lesson is the concept of dementia must always reflect the state of knowledge and is therefore in a sense ad hoc. Secondly, what the criteria for, and also who the arbiters of, the diagnosis might be is not always clear. It is traditional to think that expressing opinions and making diagnosis of mental illness is almost a civic right, i.e.

WHY WE MUST PRACTISE TRANSCULTURAL MEDICINE Health professionals and GPs should concern themselves with ethnicity, religion and culture as much as with the age, sex and social class of their patients. Transcultural medicine is the knowledge of medical and communication encounters between a doctor or health worker of one ethnic group and a patient of another. It embraces the physical, psychological and social aspects of care as well as the scientific aspects of culture, religion and ethnicity without getting involved in the politics of segregation or integration. English general practitioners and health professionals tend to regard everyone as English, and to assume that all patients have similar needs. Would that it were as simple as that! For economic reasons - based on supply and demand - the mass migration of working populations from the new Commonwealth countries, along with their dependent relatives (including their parents) to Britain took place during one decade - the 1960s. Broadly speaking, the workers were in their thirties and forties, and their dependent parents were in their fifties and sixties. All these will, of course, be 30 years older in the 1990s.

The purpose of the material in this book is to enable users of thermochemical data to predict values for standard enthalpies ofreactions involving organic compounds ranging in complex­ ity from simple alkanes to biologically important compounds such as amino acids. Chapter 1 contains tables of values for standard enthalpies of formation derived from experimental data for approximately 3000 organic compounds of the elements C, H, 0, N, S and halogens; Chapters 2 to 4 describe a simple scheme for predicting unknown values of standard enthalpies of formation. Data presented in the book are stored in a data base at the University of Sussex and with associated software provides a simple but efficient method for dealing with thermochemical problems in organic chemistry. The experimental data used in the computer calculation of the values for standard enthal­ pies of formation are clearly indicated in Table 1.2. Where alternative values for a given standard enthalpy of formation may be derived, from independent measurements, we have clearly indicated those which are regarded by the assessors as definitive and which are therefore used to derive the value for the compound concerned. We do not, however, give reasons for the assessors choice nor are details given of experimental techniques. The literature search for suitable references was discontinued in 1983 to allow development of the predictive scheme and the computer techniques for handling the data.

This volume contains the texts of invited papers presented at the Fourth International Conference on Mediterranean Ecosystems (MEDECOS) held in Perth, Western Australia during August 1984. It thus follows three previous meetings, Chile (1971), California (1977) and South Africa (1980). There has been no formal international body to organize these meetings, merely a continuity of purpose provided by the common interests of the scientists concerned in the English-speaking world. Following previous themes on structure, fire and role of nutrients in mediterranean ecosystems, MEDECOS was structured around the theme ''Resilience in Mediterranean Ecosystems''. The invited speakers were requested to deal with particu­ lar aspects of this subject, and offered papers were encouraged to do so as well. This provided a broad framework for discussions which at the same time highlighted many of the major conservation issues arising from extreme natural events and human-induced disturbances in the mediterranean regions. The proceed­ ings were issued on the last day of the conference and provided two-page accounts of each of the contributed papers and posters (Dell, B. (ed. ) 1984 Proceedings of the 4th International Conference on Mediterranean Ecosystems. Botany Dept. , University of Western Australia). This book continues the tradition of the first two editions of the late W. S. Penn''s original PVC Technology, and the extensively revised third (1971) edition prepared by myself and B. J. Lanham. In the present edition the original general format, and the arrange­ ment of chapters, have been largely preserved, but virtually nothing now remains of Penn''s own text: a part of the contents is based on material from the 1971 TitowlLanham version (revised, updated and mainly rewritten): the rest is new, including, inter alia, several chapters specially contributed by experts from the plastics industry in the UK and Europe. The section listing international (ISO) and national (BS, ASTM and DIN) standards relevant to PVC, which was first intro­ duced (as Appendix 1) in the 1971 edition, proved a popularfeature: it has now been brought up to date and considerably extended. Two further appendices provide, respectively, comprehensive unit conver­ sion"tables (with additional information on some ofthe mostfrequently encountered units, and the SI units), and a list of many properties of interest in PVC materials, with definitions, typical numerical values, and references~to relevant standard test methods. For various reasons, work on this edition involved more than the usual quota of problems: I am truly grateful to the Publisher''s Manag­ ing Editor, Mr G. B. During the past two decades, many books, governmental reports and regulations on safety measures against chemicals, fire, microbiological and radioactive hazards in laboratories have been published from various countries. These topics have also been briefly discussed in books on laboratory planning and management. The application of various scientific instruments based on different ionizing and non-ionizing radiations have brought new safety problems to the laboratory workers of today, irrespective of their scientific disciplines, be they in medicine, natural or life sciences. However, no comprehensive laboratory handbook dealing with all these hazards, some of which are recently introduced, has so far been available in a single volume. Therefore, it was thought worthwhile to publish this handbook on safety and health measures for laboratories, with contributions from several experts on these subjects. As this is a multi-author volume, some duplication in content among the chapters is unavoidable in order to maintain the context of a chapter as well as to make each chapter complete. An attempt has also been made to maintain the central theme, which is how to work in a laboratory with maximum possible environmental safety. I am indebted to Mr D. G. T. Many clinical problems of transport have been known for decades, par­ ticularly those disorders involving the liver and kidney. As a result of the dramatic increase in interest in transport at the membrane level the Society devoted its Seventeenth Symposium, held at Leeds during September 1979, to Transport and Inherited Disease, the result of that meeting forming the basis of this monograph. For the occasion over a hundred members and guests of the Society were joined by many invited speakers from Europe and the USA to discuss this rapidly developing field with special reference to the direct interests of the Society - in­ herited metabolic disease. The major theme of the meeting was opened with formal scientific presentations on membrane structure, synthesis and the regulation of epithelial transport. These were followed by discussions of specific prob­ lems of transport in brain, kidney and red blood cells. Almost all of these later lectures had clinical applications with cystic fibrosis and nephro­ genic diabetes insipidus featuring as examples of the common inherited diseases. The Hudson Memorial Lecture was delivered by Professor H. Bickel (Heidelberg). This outstanding review lecture on ''Phenylketonuri- past, present and future'' is reproduced in the Journal of the Society - the Journal of Inherited Metabolic Disease (Volume 3 No.4, pp.123-132). xiii xiv PREFACE The members'' papers (both oral and poster) are also being reprinted in various issues of the Journal (published by MTP Press Ltd., Lancaster, UK).

des Plantes (Hermann, Paris), and in 1935 by Lederer''s Les Carotenoides des Animaux (Hermann, Paris). Since then a survey such as the present one has not appeared. In order to present a full picture, much of the pre-1934 work has been reconsidered and, as far as is known, every important contribution which has appeared since that date has been discussed. Two peripheral aspects ofthe subject have, however, been omitted, namely (a) the qualita­ tive and quantitative changes which the carotenoids of plant materials undergo in storage or during processing into food and (b) the carotene (pro-vitamin A) requirements of different animal species; it was felt that the former, about which a great deal has been written, was too technological to be suitable for inclusion in the present volume, whilst the latter is more suitable for a monograph on vitamin A. The very wide distribution of the carotenoids in Nature suggests that, in spite of the superficially diverse functions ascribed to them in different living tissues, there may be some factor or property through which all these functions will eventually be correlated; any suggestion as to the nature of this common property can perhaps come most readily from a comparative approach. Apart from critically surveying the literature this book has been constructed so as to focus attention on comparative data and their possible implications.

L. B. STRANG The past 25 years have seen a remarkable growth in our knowledge of lung development in its structural, physiological and biochemical dimensions. Much of the impetus for research leading to new knowledge has derived from the perception that many respiratory disorders in the newborn infant are due to defective development or maladaption of some component or components of the respiratory system. Thus, to cite one example, surfac­ tant deficiency is clearly seen to be the cause of atelectasis in hyaline mem­ brane disease; and to cite another, it is widely accepted that the mechanisms controlling patency of the ductus arteriosus and pulmonary vascular resistance also determine the right-to-Ieft or left-to-right shunting frequently observed in the course of neonatal respiratory disorders. There are, however, areas of physiological knowledge - such as those relating to respiratory control and to liquid formation and absorption - which are clearly of great relevance to lung adaptation at birth but where it has not yet proved possible to link a specific clinical state to the malfunction of a particular mechanism. In planning this symposium an attempt was made to organize the material in an orderly manner, starting with the embryonic and fetal stages of growth and development, continuing with respiratory control and the role of surfactant in lung aeration at birth, and ending with the treatment of neonatal respiratory disorders.

This sym­ posium was held under the auspices of The Institute for Pediatric Service of Johnson and Johnson Baby Products. It was co-sponsored by The College of Medicine and Dentistry of New Jersey, Rutgers Medical School, and The In­ stitute for the Study of Exceptional Children, Educational Testing Service. The symposium was held in the Fall of 1979 in Chicago, Illinois and the participants, including Ph. D . ''s, Ed. D . ''s, and M. D . ''s, represented the interdisciplinary approach which characterized the meetings. The interdisciplinary set of speakers was matched by an equally diverse audience who represented almost every profes­ sional group concerned with the needs of disabled children and their families. This book tries to trace the course of social development in the child from birth to maturity. It assumes that the neonate comes into the world with the endowment and the need to contact others, to attach itself to the surrounding humans and by this very endowment enforces these humans to attach themselves to it. The book follows the main vicissitudes in this process and describes in short outlines some of the disturbing manifestations of failure at different ages in the smooth progress of socialization - the process by which the child adapts to the requirements of the social group of which he is a member. In accordance with recent studies the book stresses the influence on social development of the wider environment. It is no longer believed that the quality of the early mother­ child relationship exclusively determines all later develop­ ment. The book also surveys the present state of assessment and treatment methods and tries to indicate where short intervention by the developmental paediatrician might be sufficient and where more long-term intensive treatment by specialists would be needed. The author agrees with A. M. and A. D. B. Clarke (1976) that early experiences, though powerful, are not irreversible, and that with changes in environmental circumstances, and with under- 9 Social development standing of the main causal factors for the deviance, the effect of adverse early events may be modified and develop­ ment towards normal social progress may to some extent be restored.

Crises are not a feature of depressive illness; but this illness needs to be considered in the diagnoses of three acute emergencies: the agitated patient, the withdrawn patient and the suicidal patient. A. The agitated patient. Restless, anguished, phrenetic and impor­ tunate behaviour. Differential diagnoses include hypomania, acute anxiety and grief, hysteria, drug intoxication, thyrotoxicosis, cerebrovascular accident or cerebral tumour. Agitated depression carries a relatively high risk of suicide. Management usually requires admission and use of adequate doses of antidepressant and neuroleptic drugs, and often ECT. B. The withdrawn patient who avoids social contacts and obligations and is often slowed up in mind and body. Differential diagnoses in­ clude schizophrenia, CVA or tumour, hysteria and semi-coma includ­ ing drug intoxication. Withdrawn and retarded patients with depres­ sive illness are at risk of failing to eat or care for themselves. C. The suicidal patient. May present as unexpected, inexplicable coma; a badly cut patient may be confused by the doctor with acci­ dent or assault. The immediate emergency is medical or surgical: treatment is for coma, bleeding or asphyxia, and requires immediate admission to casualty. The first presentation of depression is always a minor emergency as it may be the only attempt the patient makes to see a doctor. Diagnosis must be positive, based on the recognition of depres­ sive features, not negative, based on the exclusion of other dis­ eases. The cardinal symptoms of depressive illness: 1. Disturbed sleep pattern. 2. Change in appetite for food.

The orang-utan is a superb representative of a major sector of the structure of biological diversity in Sumatra and Borneo. Conservation of the living conditions of the orang-utan implies maintaining the integrity of the entire natural ecosystem of indigenous plant and animal species known as the West Malesian rainforest, i.e. the natural tropical evergreen forests of distinctive floral composition which stretch from the isthmus of Kra, in Thailand, across peninsular West Malaysia, south and eastwards, including Sumatra, Borneo and Java. The main question behind this study is: What is the current status of the orang-utan? Or in other words: • what is the current geographical distribution range? • what have been the trends in the size of its range and numbers? • to what extent is this range covered by (a) conservation areas, (b) timher concessions (i.e. modified habitat) and (c) plans for conversion (i.e. obliteration of the habitat)? • what is the current quality of habitat in this range and what is the prospect for conservation or restoration of such habitat? • what is a plausible average density of the ape in such habitat? • what are the prospects for protection of the ape? • what should and can be clone to give the ape a chance of survival?
